What is the most common cause for a spyder gasket failure? If the valve was being
changed positions while the pump was still running would that lead to having water
being sent to waste in filter mode - by damaging the Gasket?

Yes changing the valve position while the pump is running can definitely cause this. Other possible issues that can cause this is chemical breakdown, and even if the pump ran dry for too long heating and then warping the rubber of the spider gasket. I hope this helps.
